Some services provided by Telecommunication require an update to them to work properly after a user has gone through the FRCCW process. Filling out the form lets Telecom know that the user has completed the FRCCW process and is ready for use to make those changes.
The form also lets us know what affected Telecom services the users subscribes to so we know what services to go and fix.
